Need to purchase a NFA item from us? Please fill out the following information to authorize Louisiana Firearms, LLC to complete your National Firearms Act (NFA) Responsible Person Questionnaire for you. This form is used by the United States Federal Government to verify your identity and to run a background check on you when you are attempting to purchase or create a NFA item such as a suppressor, short-barreled rifle, short barreled shotgun or machine gun as part of a Trust, Corporation or other legal entity (such as an LLC). We charge a $25 ATF Form 5320.23 filing fee per person, per item. This includes in-store fingerprinting and pictures.
